Access to your medical records

You can access your medical records through an App on your mobile:

If you would like to view your GP records and manage your repeat medications using a smart mobile or tablet device, you can download the NHS app. Please watch the short video with step by step information how to do this.

You can access your medical records on your PC or Laptop:

If you do not have a smart mobile or tablet device, you can still access your medical records and manage your repeat prescriptions by using the patient access online service via a PC or laptop. To access this you will need a linkage key (passcode) from your GP surgery.  Please ask at reception and bring photographic ID.  Once you have the information, click on the logo below to register.

What do I do if I have a query regarding the information in my medical notes?

How do I request access to my medical notes?

Patients who use an online service platform will be given automatic access to their future medical records at The Ash Surgery from 4th October 2023 in line with new Government Legislation. 

All medical records requested for dates prior to 4th October 2023 are fully read and reviewed by a G.P in the practice before access can be granted. The time taken to process this will depend upon the number of requests received and the size and complexity of the notes needed to be reviewed. 

Please be mindful of the time it takes for a G.P to process your full medical notes.  There is a section on the form to insert a date range if you feel the information you require is from a certain date onwards or within a specific time frame.  This drastically reduces the workload for your G.P.

If you do not have the ability to access your medical notes online or you are unable to see some of your information with your online access, this can also be requested.

What to do if you do NOT want online access to your medical records:

If you do not want detailed information from your medical record to be made available to you online (for example if you are worried someone might put pressure on you to see it), please let us know and we can disable this function.
